Medical Record Errors

IF YOU FIND AN ERROR IN YOUR MEDICAL RECORD, contact Health Information Management (Medical Records) at (706) 389-3360. Please, fill out the Amendment Request Form.

Please note we are able to release only records that pertain to procedures or inpatient admissions at St. Mary’s Hospital or at a St. Mary's Health Care System facility. We do not have records of visits to your physician or other healthcare providers.

Medical Records Requests


It is St. Mary's intention to comply with all applicable Federal and State laws for granting patient's access to their medical record. St. Mary's requires requests for medical records to be in writing.

Please allow us 5-7 working days after we receive your request to prepare the records. Even though State and Federal Laws give us 30 days, we will make every effort to honor your timeframe. 

Submit the Authorization for Release of Medical and/or Billing Information Form

Proper Documentation:

  • A copy of a government issued Photo ID is required with each authorization form submission. 
  • If you are the patient's representative requesting records on behalf of the patient or if the patient is deceased, documentation authorizing the release of the records to you is required. Acceptable documentation includes a letter from the patient, court order, power of attorney, executor of estate, or next of kin affidavit.

Receiving Medical Records


  • Electronic: Please provide your email address on the Authorization form and indicate that you would like delivery of your records via secure email Portal.
  • Mail: Records are typically received within 7 to 10 business days after they have been processed.
  • Continuity of Care Requests: If you wish to have records sent to a physician, please provide the physician office information, including the name and fax number, and we will release the records to your physician at no charge. Physician practices can fax a request for records to 706-389-3361 and we will send records to the physician immediately via fax. 

Fees


Fees do not typically exceed $6.50.
