Our board-certified, knowledgeable, and experienced providers offer a wide range of preventative, comprehensive gynecological, obstetrical, and surgical services to women of all ages. Clear Creek OBGYN is the practice of James Bauerband, MD, Richard White III, MD, and Amy Evans, FNP (Family Nurse Practitioner). Our physicians have an extensive combined experience of 49 years in serving the gynecological and obstetrical needs of our local communities. Dr. Bauerband has served for 26 years, and Dr. White has served for 23 years, they are joined by Amy Evans who has been a Family Nurse Practitioner for 3 years and is experienced with Emergency Room Medicine and Labor and Delivery.
Appointment Information
All patients are scheduled by appointment only. Although we make every effort to honor your appointment time, we hope you understand emergencies arise. If there are delays, we will inform you as soon as possible. If you should need to cancel or reschedule an appointment, please do so as far in advance as possible, we do require patients to notify our office within 24 hours prior to their appointment or they will be considered a no-show.
New Patients
New Patients will be required to provide the following at their appointment:
- You must arrive at your appointment no later than 30 minutes early.
- Must Provide Completed Paperwork with Health History Forms. You can complete these forms online at least 24 hours prior to your appointment. If you're unable to complete the forms online then contact our office so the registration forms can be mailed to you.
- Must provide email for Patient Portal
- Must provide Insurance cards.
- Must provide Picture ID (Driver’s License, Passport, etc.)
- Must be prepared with a form of payment (Credit/Debit Card, Cash, or Check) to pay their copays, co-insurance, or deductibles at the time of the visit.
- Must provide all Medications you are taking in their original bottles, with no exceptions.
- If your insurance requires an approval or referral for us to see you, be sure to obtain and provide this to our office before your appointment.
- If your insurance such as Tristar, HMO, POS, or other insurance that designates a specific provider on your insurance card, you will have to see that provider who is selected on your card or change it to another provider who is credentialed with that insurance before your appointment, and it must be changed in their system before we can see you.
- Adhere to the Visitor Policy- Patients are allowed 1 visitor aged 18 years or older to accompany them to their appointment. No children under 18 years of age will be permitted as a visitor. If the patient is a child under 18 years of age, they may be accompanied by one parent or guardian.
Established & Scheduled Patients
Instructions for Established Patients, Scheduled & In Office Ultrasound Patient Appointments, or Scheduled Nurse Visits:
- Must arrive at their appointments no later than 15 minutes early.
- Patients are required to provide updated information to the office regarding any change or additional information regarding address, phone number, email, other demographics, insurance, guardian, person responsible for payment, clinical history, medications, etc. to maintain accurate information in their medical record.
- Must provide all Medications you are taking in their original bottle.
