At Sacred Heart Primary Care our mission is to provide evidence-based, comprehensive care for the
entire family. Here at Sacred Heart Primary Care, we care for patients that are newborn and up. We
strive to meet not only the current healthcare needs of our patients, but practice preventative
medicine in the hope that we can prevent future disease and disability in our community.

Appointment Information
We now accept walk-in appointments!
All patients are encouraged to schedule by appointment but we do accept walk-in visits. We make every effort to honor your appointment time, we hope
you understand emergencies arise. If there are delays, we will inform you as soon as possible. If you need to cancel
or reschedule an appointment, please do so as far in advance as possible, we do require patients to notify our
office within 24 hours prior to their appointment or they will be considered a no show.
You can also fill out the patient forms online. If your insurance plan requires prior approval for services, please
be
sure to obtain your referral before your visit. Please keep us updated with any changes to your name, address,
telephone number or insurance information in order to maintain accurate information in your medical record.
- You must arrive at your appointment no later than 30 minutes early.
- Must provide completed forms, including health history forms. Patients are encouraged to
complete the online
forms at least 24 hours prior to their appointment to give our office adequate time to
review, capture, and request additional items needed for the visit. The new patient forms can be
completed on any device with internet connectivity. For patients who do not possess or
have access to the internet, registration forms will be mailed to you or in special cases will
be instructed to arrive even earlier to complete forms prior to 30 minutes before the
appointment. - Must provide email for Patient Portal.
- Must provide Insurance cards.
- Must provide Picture ID (Driver’s License, Passport, or etc.)
- Must be prepared with a form of payment (Credit/Debit Card, Cash, or Check) to pay their copays,
co-insurance, or deductibles at the time of the visit. - Must provide all Medications you are taking in their original bottles, no exception.
- If your insurance requires an approval or referral for us to see you, be sure to obtain and
provide this to our office prior to your appointment. - If your insurance such as Tristar, HMO, POS, or other insurance that designates a specific
provider on your insurance card, you will have to see that provider who is selected on your card
or change it to another provider who is credentialed with that insurance prior to your
appointment, and it must be changed in their system before we can see you. - Adhere to the Visitor Policy - Patients are allowed one visitor aged 18 years or older to
accompany them to their appointment. No children under 18 years of age will be permitted as a
visitor. If the patient is a child under 18 years of age, they may be accompanied by one parent
or guardian.
